o que faltava era esse cara aqui:

Source: C:\NotaFiscal\VB6\Instalador\PDW\Support\cxlibw-1-6.dll; DestDir: {sys}; Flags: ignoreversion recursesubdirs createallsubdirs

Ele precisava estar dentro do system para que a crqe.dll pudesse ser registrada...

Quando coloquei dentro do system ela registrou tudo certinho, passou pela instalação sem nenhum erro e o aplicativo funcionou normalmente... TUDO CERTO!
